A caged fixed ladder to a tank top or a roof hatch looks like a simple thing to order. The height that triggers a cage, the rung and cage dimensions, and whether a cage is even the right call in 2026 are less simple — and getting them wrong shows up at inspection.
Short answer: a fixed steel ladder needs fall protection above a set height, and in Ontario a safety cage is still an accepted method (O. Reg. 851) — unlike the current US OSHA rule, which is phasing cages out for ladder safety systems. As a dimensional reference, rungs run ~12" on centre, the ladder is ~16–18" wide, and a cage starts ~7–8 ft up and extends ~42" past the top landing. What a fixed ladder is — and when the cage comes in
A fixed ladder is vertical access — 75° to 90° — permanently attached to a structure: a tank, a silo, a piece of process equipment, a roof hatch. It's the most compact way up, and the least forgiving to climb, which is why the codes attach fall-protection requirements once it gets tall enough.
In Ontario, fixed ladders in industrial establishments fall under the OHSA regulation (O. Reg. 851). Above a threshold height, fall protection is required, and a safety cage is one accepted method. This is a real difference from the current US position: OSHA's 2018 rule is phasing cages out on new ladders in favour of ladder safety systems, with existing cages allowed for a transition period.
